Clinical Anchor
Diagnosis should be anchored to DSM-5-TR criteria: pervasive instability in relationships, self-image, and affect, with marked impulsivity. Five or more of the nine criteria support the symptom-count threshold, pending full clinical evaluation.
DSM-5-TR Criteria Checklist
1. Fear of abandonment: frantic efforts to avoid real or imagined abandonment.
2. Unstable relationships: intense, unstable relationships with shifts between idealization and devaluation.
3. Identity disturbance: markedly unstable self-image, values, goals, or sense of self.
4. Impulsivity: impulsive behavior in at least two potentially self-damaging areas.
5. Self-harm / suicidality: recurrent suicidal behavior, gestures, threats, or self-injurious behavior.
6. Affective instability: rapid mood reactivity or intense emotional shifts.
7. Chronic emptiness: recurring feelings of emptiness, numbness, or inner void.
8. Anger dysregulation: inappropriate, intense anger or difficulty controlling anger.
9. Stress-related paranoia or dissociation: transient suspiciousness, paranoia, unreality, or dissociation under stress.
