Symptom Severity Tracking Items

2. Fear of abandonment or rejection was activated

6. Actions occurred that were later regretted

7. Self-harm thoughts, suicidal thoughts, or crisis urges were present

11. Suspiciousness, unreality, detachment, or dissociation occurred
