Dissociation Assessment | Emotional Numbing, Derealization & Trauma Shutdown Test
Dissociation Assessment
A trauma-informed, printable self-assessment designed to help identify dissociation, emotional numbness, depersonalization, derealization, spacing out, memory gaps, shutdown responses, and trauma-related disconnection commonly associated with PTSD, Complex PTSD, anxiety, chronic stress, and nervous system overwhelm.
Psychoeducation: What Is Dissociation?
Dissociation is a protective nervous system response that can occur when emotional or physical stress feels overwhelming. Instead of moving into fight or flight, the brain may create distance from painful emotions, body sensations, memories, or the present moment. This can feel like numbness, spacing out, feeling unreal, feeling detached from the body, losing time, or emotionally shutting down.
Dissociation is not a personal failure. It is often the brain’s attempt to reduce distress when the nervous system feels overloaded. This worksheet helps identify dissociation patterns, triggers, body cues, emotional states, and grounding strategies that support reconnection to the present moment.
