PHQ-9 & GAD-7 Assessment | Depression and Anxiety Screening with Automatic Scoring
PHQ-9 & GAD-7 Depression and Anxiety Assessment
This comprehensive PHQ-9 and GAD-7 assessment combines two widely used self-report screening and symptom-severity measures for depression and anxiety. The PHQ-9 evaluates nine depressive symptoms over the previous two weeks, while the GAD-7 evaluates seven common anxiety symptoms over the same period. This updated version preserves the standard scoring structure and adds functional-impact review, safety follow-up, progress tracking, treatment-planning options, Save/Restore, printable results, and a downloadable clinical summary.
