PTSD Functional Impairment Assessment | Trauma, C-PTSD & Daily Functioning Evaluation
PTSD Functional Impairment Assessment
A trauma-informed, printable assessment designed to evaluate how PTSD, Complex PTSD, emotional flashbacks, hypervigilance, dissociation, anxiety, and trauma-related stress responses affect daily functioning, work performance, relationships, emotional regulation, sleep, concentration, and overall quality of life.
Psychoeducation: Functional Impact of Trauma
Trauma does not only affect emotions and memories. PTSD and Complex PTSD can significantly impact day-to-day functioning, including concentration, sleep, communication, parenting, work performance, emotional regulation, trust, social engagement, motivation, physical health, and the ability to feel safe or emotionally present.
Many individuals with trauma-related symptoms continue functioning outwardly while internally struggling with chronic stress activation, emotional exhaustion, hypervigilance, shutdown responses, emotional flashbacks, dissociation, or avoidance behaviors. This worksheet helps identify areas of functional impairment so that treatment goals, coping strategies, accommodations, and recovery plans can be better targeted.
